New Immunization Requirements for 2013 Yorkville School Registration

Registration for the 2013-14 school year will take place on Monday, Aug. 5th and Tuesday, Aug. 6th.

In accordance with the Illinois Department of Public Health, Yorkville schools will require students registering for 6th through 12th grades to show proof of receipt of one dose of Tdap vaccine (combned tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTaP, DT or Td dose.

If a student has already received the vaccine, they will merely be required to show documentation to verify. According to the Illinois Department of Public Health, the following documents are acceptable proof of vaccination:

  • Note or letter, signed by health care kprovider and identifying the vaccine (Tdap) and date (month, day, and year) administered.
  • Print-out from provider’s electronic medical record system that identifies Tdap vaccination(s) and date administered for student in question.
  • Current Certificate of Child Health Examination, specifying Tdap and date (month, day, year) administered.

Students without proof of vaccination will not be allowed to start school on August 14th, 2013. Parents are encouraged to check with their health care provider to confirm that they are compliant.
