New Immunization Required for Yorkville 115 Students

Students without proof of the Tdap vaccination will not be allowed to start school on August 14, 2013.

The following is from a release issued by the Yorkville 115 School District:

In accordance with the Illinois Department of Public Health, Yorkville schools will require students registering for 6 through 12 grades to show proof of receipt of one dose of Tdap vaccine (combned tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTaP, DT or Td dose.

If a student has already received the vaccine, they will merely be required to show documentation to verify. According to the Illinois Department of Public Health, the following documents are acceptable proof of vaccination:

  • Note or letter, signed by health care provider and identifying the vaccine (Tdap) and date (month, day, and year) administered. 
  • Print-out from provider’s electronic medical record system that identifies Tdap vaccination(s) and date administered for student in question. 
  • Current Certificate of Child Health Examination, specifying Tdap and date (month, day, year) administered. 

Students without proof of vaccination will not be allowed to start school on August 14, 2013.

Parents are encouraged to check with their health care provider to confirm that they are compliant. All questions regarding the new immunization requirements should be directed to Maribeth Surma, Health Services Coordinator for Yorkville School District 115 (630)553-4385 extension 4405.
